[ietf-nntp] Issues from news.software.nntp

About 2 months ago Russ posted the entire text to news.software.nntp. I
used the responses to make some minor edits to draft 21, and kept those
articles that raised bigger issues. I've now got round to looking at the
latter.

So that we don't get tangled, I'm going to post each issue in a separate
email for people to discuss.
