The updated document has finally been updated. Many of the changes that have
been discussed have been included. I have already posted some sections that
have been changed. In addition, the following changes have been done:

Changes in 7.1.2 to clarify that the response from the server is not limited to
20X responses.

Changes in section 9.5.3.1.2 to fix some mistakes in the examples in response to
recent comments on the list from Tim Roberts.


Things that have not changed:

Section 5 has not been changed.

Streaming is not explicitly discussed.
